Medical Sickness Certificates ("Lines")

 

You are responsible for self-certification for the first SEVEN DAYS of any illness (including weekends and public holidays).

If you have been ill for 7 days or less, the GP will be unable to issue you a sick line.

You can complete a Self-Certification (SC2) online.


Many employers have their own self-certification forms so please speak to your employer about this. Emergency or urgent appointments are not to be used to request Sick / Fit notes. Sickness certificates can be backdated so please discuss this when you speak to the practice. It is important that the GP is told the correct dates you are requiring a Sickness certificated.
