Medical Sickness Certificates ("Lines")

You are responsible for self-certification for the first SEVEN DAYS of any illness (including weekends and public holidays).

If you have been ill for 7 days or less, the GP will be unable to issue you a sick line.

You can complete a Self-Certification (SC2) please see link below.

Many employers have their own self-certification forms.

If your employer doesn’t have its own form, you can download a Self-Certification Form (PDF) via the link below. Please print it, fill it in and hand it in to your employer.

Emergency or urgent appointments are not be used to request Sick / Fit notes.

Please note a Sick or Fit Note can be backdated; please discuss this when you speak to the practice.

NB:

1. You only require a 'Fit Note' after 7 days self-certification

2. You only require a 'Fit Note' after 7 days and only if you are in employment

3. It is important that the GP is told 'from when and until when' you are requiring a 'Fit Note'
